Today we’d like to introduce you to Scarlett Idema.
Scarlett, we appreciate you taking the time to share your story with us today. Where does your story begin?
I was born in Costa Rica as the eighth of ten children, in a family where resilience wasn’t something you talked about, it was just how you lived. Resources were limited, but work ethic, faith, and determination were abundant. From a young age, I learned how to take care of others, how to adapt, and how to keep moving forward no matter the circumstances.
I immigrated to the United States at 19, starting from scratch in a completely new environment. Like many immigrants, I took the opportunities that were available to me at the time and built from there. I spent over 20 years in cosmetology, which gave me something incredibly valuable beyond income, it gave me deep, personal connections with people. I learned how to listen, how to earn trust, and how to show up consistently for my community.
Over time, I felt called to do more on the clinical side of care. That decision wasn’t easy. Going back to school while working, raising a family, and managing life responsibilities required discipline and sacrifice. But I stayed focused on the long-term vision. I transitioned into nursing, and eventually became a Family Nurse Practitioner.
Today, I own and operate a clinic in Detroit. What I’ve built is not just a healthcare business—it’s a community-centered space where patients feel seen, heard, and cared for in a way that’s often missing in traditional systems. My goal has always been to bridge gaps in access and provide high-quality care with dignity, especially for underserved populations.
Detroit has played a huge role in my journey. This city understands resilience, reinvention, and grit, and that aligns deeply with who I am. Being able to serve this community, create jobs, and expand access to care is something I don’t take lightly.
At the core of it all, I’m still that same person who believes in showing up, working hard, and taking care of people. The difference now is that I’ve been able to build something bigger than myself—something that hopefully continues to grow and impact lives for years to come.
I’m sure it wasn’t obstacle-free, but would you say the journey has been fairly smooth so far?
Not at all—it’s been anything but smooth.
Coming to the U.S. at 19, I had to learn how to navigate a completely new system without a roadmap. There were financial challenges, long work hours, and the constant pressure of figuring things out on my own. I worked while building a life here, and nothing came quickly or easily.
Transitioning careers was one of the biggest challenges. After spending over 20 years in cosmetology, going back to school for nursing meant starting over in many ways. Balancing school, full time work, and single motherhood required a level of discipline and sacrifice that pushed me to my limits. There were moments of doubt, times when the path forward didn’t feel clear but I stayed committed to the bigger picture.
Even after becoming a Family Nurse Practitioner, the challenges didn’t stop. Opening and running a clinic in Detroit came with its own set of obstacles such as navigating regulations, dealing with insurance systems, managing staffing, and building trust within the community. As a business owner in healthcare, you’re constantly balancing patient care with the realities of running an operation.
There were also moments where I had to advocate for myself in spaces where I wasn’t always expected to lead. That takes confidence, persistence, and a willingness to keep showing up, even when things feel uncertain.
But those challenges shaped everything. They forced me to grow, to adapt, and to stay focused on why I started in the first place. It hasn’t been a straight line, but every obstacle played a role in getting me to where I am today.
Alright, so let’s switch gears a bit and talk business. What should we know?
Medicina Scarlett is a community-centered healthcare clinic in Detroit focused on delivering accessible, high-quality care with a personal touch. We operate as both a primary care and same day care which allows us to meet patients where they are, whether they need routine management of chronic conditions or same-day care for acute issues.
What we’re known for is how we treat people. Patients are not rushed, dismissed, or treated like a number. We take time to listen, educate, and build real relationships. That approach has allowed us to serve a diverse population, including many underserved and Spanish-speaking patients who often face barriers in traditional healthcare settings.
Clinically, we specialize in chronic disease management: things like diabetes, hypertension, and metabolic conditions but we also focus heavily on preventive care and early intervention. We’ve also integrated services like weight management, hormone optimization, and in-office treatments such as iron infusions to help patients avoid unnecessary hospital visits or long wait times for those same services.
What sets us apart is the combination of accessibility and depth of care. We are a walk-in friendly clinic, but we don’t sacrifice quality for volume. We’re building a model where patients can receive comprehensive care in one place, without long wait times or fragmented systems.
Brand-wise, what I’m most proud of is the trust we’ve built in the community. Patients come back, they refer their families, and they feel comfortable here. That’s not something you can buy, it’s earned over time through consistency and genuine care.
We’re also in the process of expanding into a much larger facility in Detroit, which will allow us to offer even more services, including imaging and expanded urgent care capabilities. The goal is to continue growing without losing the core of who we are: a clinic built on compassion, respect, and high standards of care.
What I want readers to know is that Medicina Scarlett is more than a clinic, it’s a place where people can feel taken care of in a way that’s often missing in today’s healthcare system.
How do you define success?
Success, to me, is impact.
It’s not just about growth, revenue, or expansion; those things matter, but they’re not the core of it. Success is when patients trust us enough to come back, bring their families, and feel genuinely cared for. It’s when someone’s health improves because we took the time to listen and do things the right way.
It’s also about creating opportunities. Being able to build something in Detroit that provides jobs, develops people, and creates a strong team culture—that’s a big part of success for me. I want the people who work with me to grow, to feel proud of what they do, and to see a future for themselves within the organization.
On a personal level, success means staying aligned with why I started. If I can continue to serve the community, maintain high standards of care, and grow without losing that human connection, then I know I’m on the right path.
At the end of the day, success is building something that makes a real difference; and doing it in a way that lasts.
